Chapman university has discontinued the blanket requirement that all full-time undergraduate students provide evidence of health insurance. This is effective with the start of the 2012-2013 academic year and is consistent with the individual mandate under the Patient Protection and Affordable Care Act(PPACA) that became effective March 23, 2010.

International undergraduates or graduates who are required to provide evidence of insurance under the F1/J1 Visa requirements or one of the following: Physical Therapy, Pharmacy, Physicians Assistant students who are required to provide evidence of coverage under the terms of a clinical affiliation agreement. If you fall under these categories of students, then you must provide confirmation that you have the required coverage, or you will be enrolled in a plan selected by the University and charged the cost of coverage.
The requirements of the F1/J1 Visa are:
Medical Benefits of at least $100,000 per accident or illness
Repatriaton of Remains in the amount of $25,500
Expenses associated with the medical evacuation of the exchange visitor to his or her home country in the amount of $50,000
A deductible not to exceed $500 per accident or illness
PHYSICAL THERAPY STUDENTS
The requirements of your clinical practice affiliation agreement stipulate that the student will provide evidence of health insurance coverage at the beginning of the clinical experience.
Upon successful completion of this form you will exit to “You have successfully submitted your waiver” page.
You will receive an email sent to your Chapman email account confirming successful submission of this form. If you do not get the email please contact the Health Center.
Submissions are audited. Submitting false information could subject you to disciplinary action.
